A health declaration form is a short questionnaire that asks about your health and recent movements before a country lets you in. Border and public health officials use it to spot travelers who may need a check on arrival.

Most versions ask for your passport details, your flight number, the countries you visited in the last week or two, where you are staying and whether you have symptoms such as fever, a cough or a rash. When you actually need one

Health declaration, customs declaration and vaccination certificate

These three get mixed up constantly, and they are separate documents that answer separate questions.

Health declaration

Covers your current health and recent travel history. It exists to manage disease risk and the official version is almost always free.

Customs declaration

Covers what you are bringing in: cash above a threshold, goods, food, plants and alcohol. It has nothing to do with your health.

Vaccination certificate

Proof that you had a specific vaccine, most often yellow fever. Some countries refuse entry without it if you arrived from a risk area, which a health declaration alone will not fix.

How to fill one in without problems

  1. Use the official government portal. Lookalike sites charge a fee for a form that is free.
  2. Match the spelling of your name to your passport exactly, including middle names.
  3. Submit inside the country's stated window, often 24 to 72 hours before arrival. Filing too early can invalidate the form.
  4. Have your flight number, seat number and accommodation address ready before you start.
  5. Complete a separate form for each traveler unless the portal offers a family submission.

Important: A missing health declaration can stop you at the gate, not just at the border. Airlines are fined for carrying passengers who are refused entry, so check-in staff may deny boarding if the destination requires a form and you cannot show it.

What hotels ask for

Hotels do not collect health declarations. At check-in they want photo ID and your booking confirmation, and in many countries they register guests with local authorities using your passport details. Your trip1 confirmation is what they scan or type in, so have it on your phone.
